Disclosures and Consent to Use of Electronic Signatures
- By electronically signing these documents, you are agreeing to complete and submit the prepaid funeral contract and the application for coverage by way of an electronic transaction, including the use of electronic signatures, and to receive electronic versions of certain records.
- You are further agreeing that your consent will be as legally binding and enforceable as if you had signed on paper.
- You are entitled to change your mind and withdraw your consent by notifying the Company in writing at customer.service@funeraldirectorslife.com. Your consent will remain in effect until the Company is notified that it is withdrawn.
- If you consent to electronic delivery of the required information and then decline the e-signature process, the Company cannot continue processing your contract and application electronically.
- Your consent applies to all required information the Company gives you, or receives from you, about your contract and application, including any notices, disclosures, authorizations, acknowledgements, or other documents related to it.
- Once your application is processed and finalized, you may review your documents online at any time by creating an account at www.fdliconline.com.
- You agree to keep us informed of any change in your e-mail or mailing address during the eSignature process.
Your agreement to the eSignature consent, provided hereafter, confirms the following:
- I can access and read this "eSignature Consent"; and
- If applicable, I have an e-mail service provider and I am able to send e-mail and receive e-mail with hyperlinks to web sites to attached files; and
- If applicable, until or unless I notify the company as described above, I consent to receive all required notices and disclosures relating to my contract and insurance applicable exclusively through electronic means;
- I have the right to receive hard copy documents upon request by emailing customer.service@funeraldirectorslife.com; and
- If applicable, I also consent to the use of electronic signatures in connection with my contract and insurance applicable with the Company in place of handwritten signatures.
